Constructivist research in Science and Technology Studies is committed to revealing the heterogeneity of technological change and the fluid boundaries between the elements involved. Its major theories, the Social Construction of Technology and Actor Network Theory, have however both been criticized for limiting themselves to the micro-level of cases, impeding a structural analysis of technological systems. This article seeks to bridge any such divides. We research the recent changes in the viticulture of Mendoza, Argentina, which underwent radical changes over the (...) past decades: once governed by low-cost yield maximization, grape production now revolves around “quality.” To show how the particular quality definition developed, we depart from a social-constructivist framework, following the sociotechnical shaping of problem-solution relationships across the wine production system. To include relevant social groups from different settings around the world, we gradually incorporate the ANT concepts of obligatory passage points, enrollment, convergence, delegation, and codes into the analysis. Combined into the concept of “sociotechnical alliance,” our analysis follows the dual process of creating agreement while establishing heterogeneous practices across settings at different levels. It shows that functioning involves alliance building and highlights the hybridity and continuous dynamics of systems at large. (shrink)

The concept of cooperative communities that enforce norm conformity through reward, as well as shaming, ridicule, and ostracism, has been central to anthropology since the work of Durkheim. Prevailing approaches from evolutionary theory explain the willingness to exert sanctions to enforce norms as self-interested behavior, while recent experimental studies suggest that altruistic rewarding and punishing—“strong reciprocity”—play an important role in promoting cooperation. This paper will use data from 308 conversations among the Ju/’hoansi (!Kung) Bushmen (a) to examine the dynamics of (...) norm enforcement, (b) to evaluate the costs of punishment in a forager society and understand how they are reduced, and (c) to determine whether hypotheses that center on individual self-interest provide sufficient explanations for bearing the costs of norm enforcement, or whether there is evidence for strong reciprocity. Auguste Comte et William Whewell sont contemporains. Tous les deux ont écrit sur de nombreux sujets : histoire et philosophie des sciences, astronomie, mécanique, philosophie morale et réflexions sur l’éducation. Leurs œuvres de philosophie des sciences sont parmi les plus importantes du XIXe siècle. Mais si celle du premier a marqué le siècle, celle du second n’a fait que de timides apparitions. C’est probablement par John Stuart Mill, à la fois disciple et sectateur de Comte et farouche opposant de Whewell, (...) que les deux philosophes ont connu chacun l’œuvre de l’autre. Leurs œuvres présentent des ressemblances dans la démarche et dans la méthode mais elles s’opposent sur de nombreuses questions de fond. On cherche à montrer la grandeur du philosophe britannique à qui l’histoire n’a pas fait la place que l’immensité de son œuvre aurait dû lui valoir.Auguste Comte and William Whewell are contemporaries. Both wrote on numerous topics : i.e. history, philosophy of sciences astronomy, mechanics, moral philosophy and thoughts on education. Their works are among the most important ones of the 19th century. But whereas those of the former left their mark on the century, those of the latter were but rather shyly prominent. It is most probably through John Stuart Mill, both a disciple and worshipper of Comte’s whilst a fierce opponent of Whewell’s that both philosophers both came across each other’s works. Both their works are similar in the methodological approach but they differ greatly on fundamental topics. PDZ (also called DHR or GLGF) domains are found in diverse membraneassociated proteins including members of the MAGUK family of guanylate kinase homologues, several protein phosphatases and kinases, neuronal nitric oxide synthase, and several dystrophin‐associated proteins, collectively known as syntrophins. Many PDZ domain‐containing proteins appear to be localised to highly specialised submembranous sites, suggesting their participation in cellular junction formation, receptor or channel clustering, and intracellular signalling events. PDZ domains of several MAGUKs interact with the C‐terminal polypeptides of a subset (...) of NMDA receptor subunits and/or with Shaker‐type K+ channels. Other PDZ domains have been shown to bind similar ligands of other transmembrane receptors. Recently, the crystal structures of PDZ domains, with and without ligand, have been determined. These demonstrate the mode of ligand‐binding and the structural bases for sequence conservation among diverse PDZ domains. (shrink)

People who have not experienced diseases and health conditions tend to judge them to be worse than they are reported to be by people who have experienced them. This phenomenon, known as the disability paradox, presents a challenge for health policy, and in particular, healthcare resource distribution. This divergence between patient and public preferences is most plausibly explained as a result of hedonic adaptation, a widespread phenomenon in which people tend to adapt fairly quickly to the state they are in, (...) good or bad, and adjust their baseline utility accordingly.If patient utilities can be shown to be inappropriate for use in public policy decision making, the disability paradox fades away. This paper offers a critique of one such attempt: the idea that adaptation leads to adaptive preferences. I argue that none of the main accounts of adaptive preferences in fact characterise adapted patient preferences as irrational. Adapted preferences should not, therefore, be treated as synonymous with adaptive preferences. I suggest that much patient adaptation should be understood as a form of the ubiquitous human ability to respond to environmental change. Consequently, we ought not to discount patient preferences. This book provides a major new cross-disciplinary framework for thinking about poverty and human rights. Drawing on the fields of ethics, economics, and international law, Vizard demonstrates how the work of Nobel Laureate Amartya Sen has expanded and deepened human rights discourse across traditional disciplinary divides.

It is a commonly expressed sentiment that the science and philosophy of well-being would do well to learn from each other. Typically such calls identify mistakes and bad practices on both sides that would be remedied if scientists picked the right bit of philosophy and philosophers picked the right bit of science. We argue that the differences between philosophers and scientists thinking about well-being are more difficult to reconcile than such calls suggest, and that pluralism is central to this task. (...) Pluralism is a stance that explicitly drives towards accommodating and nurturing the richness and diversity of well-being, both as a concept and as an object of inquiry. We show that well-being science manifests a contingent pluralism at the level of methodology, whereas philosophy of well-being has largely rejected pluralism at the conceptual level. Recently, things have begun to change. Within philosophy, conceptual monism is under attack. But so is methodological pluralism within science. We welcome the first development, and bemoan the second. We argue that a joined-up philosophy and science of well-being should recognise the virtues of both conceptual and methodological pluralism. Philosophers should embrace the methodological justification of pluralism that can be found in the well-being sciences, and scientists should embrace the conceptual reasons to be pluralist that can be found in philosophical debate. The cultural evolutionary approach to the dynamics of cumulative culture is insufficient for understanding how culture affects heritability estimates; it ignores the agency of individuals and internal complexity of social groups that drive cultural evolution. Both environmental and social selection need consideration. The WEIRD (western, educated, industrialized, rich, and democratic) problem has never plagued anthropology: A wealth of ethnography is available for the problem at hand.

While not a coherent body of thought, the socialist ideology of the Panhellenic Socialist Movement (PASOK) and its leader, Andreas Papandreou, diverges from the socialist principles of post-World War II social democratic parties of Western Europe. The latter spoke for the working class in advanced industrialized societies. These social democrats accepted the legitimacy of capitalism and, assuming continued economic growth, advocated reformist policies. Such programs were meant to strengthen the welfare state and bring about a more equitable distribution of income. (...) As social democracy began to falter, its Keynesian theoretical presuppositions became transparently inadequate. Social democracy's ideological moorings were adrift, while its policies became barely distinguishable from those of its conservative political opponents. (shrink)

Patient safety is a central aspect of healthcare quality, focusing on preventable, iatrogenic harm. Harm, in this context, is typically assumed to mean physical injury to patients, often caused by technical error. However, some contributions to the patient safety literature have argued that disrespectful behavior towards patients can cause harm, even when it does not lead to physical injury. This paper investigates the nature of such dignitary harms and explores whether they should be included within the scope of patient safety (...) as a field of practice. We argue that dignitary harms in health care are—at least sometimes—preventable, iatrogenic harms. While we caution against including dignitary harms within the scope of patient safety just because they are relevantly similar to other iatrogenic harms, we suggest that thinking about dignitary harms can help to elucidate the value of patient safety, and to illuminate the evolving relationship between safety and quality. In his 1987 paper “Truth or Consequences,” Dan Brock describes a deep conflict between the goals and virtues of philosophical scholarship and public policymaking: whereas the former is concerned with the search for truth, the latter must primarily be concerned with promoting good consequences. When philosophers are engaged in policymaking, he argues, they must shift their primary goal from truth to consequences—but this has both moral and methodological costs. Brock’s argument exemplifies a pessimistic, but not uncommon, view of the possible (...) shape and nature of applied philosophy. The present paper paints a richer and more optimistic picture. It argues that the difference between theoretical philosophy and applied philosophy is not best understood as a choice between truth and consequences. On the contrary, applied philosophers engage in forms of truth‐seeking that are properly concerned with consequences—including the consequences of philosophical practice itself. (shrink)

This article investigates the theoretical assumptions and implications of de Ste. Croix’s approach to interstate politics in The Origins of the Peloponnesian War. It suggests that two approaches can be identified in the work: one which sees a fundamental connection between political systems within a state and that state’s conduct of interstate politics, and another, closer to conventional ‘Realist’ theories, which sees a clear dividing line between domestic and interstate politics, and in which interstate relations need to be understood according (...) to a distinct analytical framework. Although this tension was probably not a particular concern to de Ste. Croix himself, it does have a bearing on ongoing debates in International Theory; the final part of the article briefly explores the possibility that the concept of ‘compulsion’, important to both Thucydides’ and de Ste. Croix’s understanding of the causes of the Peloponnesian War, might provide a way of reconciling these two approaches. (shrink)

This study investigates whether the host personality and the primary alterpersonality of a woman with multiple personality disorder are controlled by the left and right hemispheres, respectively. Results support the hypothesis. Behavioral and preference measures indicate that Pe is strongly right handed and Pa is left handed. Verbal and musical dichotic tests show significantly greater accuracy for stimuli presented to the left ear for Pa and to the right ear for Pe. It is concluded that shifts in hemisphericity involve redistribution (...) of attentional resources and callosal suppression. “Conditional handedness” is proposed as a handedness subtype, characterizing persons who alternate personalities and consistently display alternate hand preferences linked to specific personality characteristics. Este artigo tem como finalidade analisar aspectos teóricos comuns entre o historiador inglês Edward Palmer Thompson e o educador brasileiro Paulo Freire, como a crítica ao idealismo marxista desvinculado da experiência cultural dos sujeitos e a consequente negação de seu papel na história. Não há a pretensão de incluir os dois autores em um mesmo campo teórico, um marxismo, mas destacar que a cultura - enquanto modos de vida e experiência em Thompson – e como leitura de mundo em Freire, (...) se constitui como fundamento da historiografia social e da educação popular humanizadora. Person-centred care is a cornerstone of contemporary health policy, research and practice. However, many researchers and practitioners worry that it lacks a 'clear definition and method of measurement,' and that this creates problems for the implementation of person-centred care and limits understanding of its benefits. In this paper we urge caution about this concern and resist calls for a clear, settled definition and measurement approach. We develop a philosophical and conceptual analysis which is grounded in the body of literature concerning (...) the theory and practice of person-centred care. We consider a range of influential definitional frameworks of person-centred care, highlighting their differences and showing that they do not correspond to a clearly circumscribed and consistent underlying concept. We argue that a degree of indeterminacy and vagueness should not be seen as a problem with the concept of person-centred care; these are features of a rich and contested concept which exists prior to and outside of practical and technical operational definitions and applications. We defend the value of operating with multiple accounts of person-centred care, arguing that what counts as being person-centred can vary across different care contexts, in relation to different patient groups, and as a reflection of different, defensible ethical perspectives. Although the idea of a single, agreed definition is attractive and may seem to be a practical or even necessary step towards meaningful and coordinated action, we argue that this is only the case in a qualified sense. Comprehensive attempts to narrow down the concept in this way should be resisted, as they risk undermining what it is that makes person-centredness a valuable concept in healthcare. (shrink)

In the twenty-first century, there has been a resurgence of ecologically conscious art among artists using new technologies. Like Eco-art, this recent movement, which might be called Eco-media, is interdisciplinary. Eco-media is heavily influenced by developments in environmental science, in particular developments in remote imaging and other kinds of remote Earth sensing (for example, the widespread use of satellite imaging and GPS) and developments in computer modelling (for example, detailed global models of climate that not only model the physics of (...) the Earth and solar system, but also explore the chemistry and biology of the Earth). This article will discuss developments in media art related to the computerized interpretation of complex data sets describing and modelling the natural world. It will explore work that has historically straddled artistic and scientific development, specifically Marey's work in the nineteenth century in photography and motion pictures and give examples of artists who are working in the area in-between art and science today. It will also discuss connections between this work and the historical Romantic sublime and the political dimension of this work as advocating for the preservation of the natural world, and give examples of some of the author's artworks in this area. Evaluation, as the expression of a writer’s attitudes, opinions and values, has become a key term in discourse studies in recent years and has proved to be a particularly fruitful way of analysing academic texts. But while studies have shown the importance of evaluation in research genres, its role in seemingly more promotional academic genres has been largely neglected. This article examines the journal description, a brief but ubiquitous feature of all journals, whether online or in print. Situated at the (...) academic—commercial interface, the JD provides information for prospective readers and authors while endorsing a particular view of the field and positioning the journal in the academic community. Drawing on a corpus of 200 JDs in four contrasting disciplines, we show how evaluation is a key feature of this genre, influencing both lexical choices and rhetorical structure. The analysis contributes both to our understanding of a neglected academic genre and the evaluative resources of language. (shrink)

Neste trabalho apresentamos alguns resultados da pesquisa de doutorado desenvolvida junto à faculdade de Educação da Universidade de São Paulo entre 2002 e 2006, com o título: Ética do discurso e Ética Universal do Ser Humano: convergências entre Paulo Freire e Jurgen Habermas. Trata-se de uma breve descrição dos resultados obtidos, no intuito de aproximar o pensamento de dois autores complexos e de grande importância no pensamento filosófico mundial. Por um lado temos Paulo Freire e sua preocupação em relacionar educação (...) e crítica social libertadora. Por outro, o esforço gigantesco de Jürgen Habermas em produzir uma grande interpretação sobre o mundo, defendendo o projeto inicial da modernidade. Ambos se aproximam no final da década de 1990, chegando quase a um encontro pessoal, no âmbito da Teoria Crítica da Sociedade, quando seguidores de lado a lado identificam conceitos e intenções próximos, como a ideia do diálogo qualificado, de uma ética universalista possível, de uma emancipação humana no horizonte. Este trabalho tem como propósito contribuir para a desmistificação das atribuições pejorativas sugeridas por diferentes atores do campo não progressista, sustentadas em desinformação e falta de referenciais sólidos de análise, sobre o sentido emancipatório da obra de Paulo Freire e suas incidências sobre a educação em direitos humanos. Também tem o objetivo de analisar as sugestões teóricas de Freire como caminhos para a construção de mecanismos de consolidação de uma radicalidade democrática no seio da sociedade brasileira. Setores da sociedade sugerem (...) que Paulo Freire seria um autor de vertente marxista, postulador de doutrinas ortodoxas “esquerdistas” nos processos de educação formal, justificando propostas cujo teor têm como objetivo a contraposição a esta suposta doutrinação, como o projeto denominado “Escola sem partido”. Buscamos demonstrar que Paulo Freire tem como preocupação primeira o de caráter ontológico nos processos educativos, a formação humana, indicando meios educacionais para o aprimoramento da reflexão e da ação intimamente relacionadas, que levem ao aprimoramento do ser humano e que, no bojo desta preocupação, existe um objetivo maior, emancipatório, comportando todas as ordens deste aprimoramento: a promoção da justiça, o caráter laico e universal da escola pública, o direito à livre manifestação de ideias, o respeito às realidades culturais de cada estudante, a construção coletiva do conhecimento e a defesa dos direitos humanos. The key synergy is that at time scales of about 1/3 of a second, the natural sequentiality of body movements can (...) be matched to the natural computational economies of sequential decision systems through a system of implicit reference called deictic in which pointing movements are used to bind objects in the world to cognitive programs. This target article focuses on how deictic bindings make it possible to perform natural tasks. Deictic computation provides a mechanism for representing the essential features that link external sensory data with internal cognitive programs and motor actions. An experimental realization of quantum computers is composed of two or more calcium ions trapped in a magnetic quadripole. Information is transferred to and read from the ions by means of structured lasers that interact with the ions’ vibration pattern, causing changes of energy distribution in their electronic structure. Departing from an initial state when the ions are cooled, the use of lasers modifies the internal state of one ion that is entangled with the others, then changing the collective states. (...) In such quantum computers, some of the physically possible electronic states are avoided or not taken into consideration, to force the system to work as a binary device. Carina Fourie and Annette Rid’s edited volume What Is Enough? Sufficiency, Justice, and Health comprises fifteen original contributions which explore the possibility of a sufficientarian approach to healthcare priority setting and resource allocation. Sufficientarianism is a well-established theory of distributive justice, which tells us that justice requires that each person has “enough,” and assigns particular importance to a threshold level of goods under which no person must fall. Sufficiency is under-explored as a distributive principle in the healthcare context, and this (...) book makes a strong case for its inclusion among more familiar principles of justice such as utility, priority to the worst off, and equality. Buddhism first came to the West many centuries ago through the Greeks, who also influenced some of the culture and practices of Indian Buddhism. As Buddhism has spread beyond India, it has always been affected by the indigenous traditions of its new homes. When Buddhism appeared in America and Europe in the 1950s and 1960s, it encountered contemporary psychology and psychotherapy, rather than religious traditions. Since the 1990s, many efforts have been made by Westerners to analyze and integrate the similarities (...) and differences between Buddhism and it therapeutic ancestors, particularly Jungian psychology. Taking Japanese Zen-Buddhism as its starting point, this volume is a collection of critiques, commentaries, and histories about a particular meeting of Buddhism and psychology. It is based on the Zen Buddhism and Psychotherapy conference that took place in Kyoto, Japan, in 1999, expanded by additional papers, and includes: new perspectives on Buddhism and psychology, East and West cautions and insights about potential confusions traditional ideas in a new light. It also features a new translation of the conversation between Schin'ichi Hisamatsu and Carl Jung which took place in 1958. _Awakening and Insight_ expresses a meeting of minds, Japanese and Western, in a way that opens new questions about and sheds new light on our subjective lives. In this paper, we argue that there are important ethical questions about healthcare improvement which are underexplored. We start by drawing on two existing literatures: first, the prevailing, primarily governance-oriented, application of ethics to healthcare ‘quality improvement’ (QI), and second, the application of QI to healthcare ethics. We show that these are insufficient for ethical analysis of healthcare improvement. In pursuit of a broader agenda for an ethics of healthcare improvement, we note that QI and ethics can, in some respects, (...) be treated as closely related concerns and not simply as externally related agendas. To support our argument, we explore the gap between ‘quality’ and ‘ethics’ discourses and ask about the possible differences between ‘good quality healthcare’ and ‘good healthcare’. We suggest that the word ‘quality’ both adds to and subtracts from the idea of ‘good healthcare’, and in particular that the technicist inflection of quality discourses needs to be set in the context of broader conceptualisations of healthcare improvement. We introduce the distinction between quality as a measurable property and quality as an evaluative judgement, suggesting that a core, but neglected, question for an ethics of healthcare improvement is striking the balance between these two conceptions of quality. ABSTRACT: Efficiency is often overlooked as an ethical value and seen as ethically relevant chiefly when it conflicts with other values, such as equality. This article argues that efficiency is a rich and philosophically interesting concept deserving of independent normative examination. Drawing on a detailed healthcare case study, we argue that making assessments of efficiency involves value-laden, deliberative judgments about how to characterize the functioning of human systems. Personal and emotional resources and ends are crucial to system functioning but are (...) often discounted in favor of a relatively narrow set of financial inputs and institutional or procedural outputs. Judgments about efficiency tend to advantage (or disadvantage) different parties, depending on the resources and ends considered. Different constructions of efficiency can therefore promote or neglect the perspectives and interests of differently placed actors. Models of efficiency do not merely embody contestable ethical standpoints but—put to use—can unwittingly reify and reproduce them. (shrink)

Disrespect in health care often persists despite firm commitments to respectful service provision. This conceptual paper highlights how the ways in which respect and disrespect are characterised can have practical implications for how well disrespect can be tackled. We stress the need to focus explicitly on disrespect (not only respect) and propose that disrespect can usefully be understood as a failure to relate to people as equals. This characterisation is consonant with some accounts of respect but sometimes obscured by a (...) focus on respecting people’s autonomy and dignity. Emphasising equality is consistent with connections patients draw between being (dis)respected and (in)equality. It readily accommodates microaggressions as forms of disrespect, helping to understand how and why experiences of disrespect may be unintentional and to explain why even small instances of disrespect are wrong. Our view of disrespect with an emphasis on equality strengthens the demand that health systems take disrespect seriously as a problem of social injustice and tackle it at institutional, not just individual levels. It suggests several strategies for practical action. Emphasising relational equality is not an easy or short-term fix for disrespect, but it signals a direction of travel towards an important improvement ambition. (shrink)

Considéré comme véhicule du processus de la sémiose, cette notion s’interpose au cœur de la réflexion sémiotique comme instance chargée de faire passer le sens. L’espace de la ville, ses composants et (...) les pratiques qui s’y déroulent sont sollicités dans cette étude, comme point d’articulation de notre problématique qui consiste à savoir comment le pont en tant qu’élément architectural dans l’espace d’une ville, par son statut, les pratiques qui s’y passent et les valeurs qu’il porte, pourra fonctionner comme médiateur sémiotique. Nous tenterons de montrer à travers l’étude de notre corpus, le pont Khadjou à Ispahan, ville iranienne, comment ce rôle médiateur s’implante sur le niveau cognitif autant que passionnel des sujets qui le côtoient. The majority of commentators agree that the time to focus on embodiment has arrived and that the disembodied approach that was taken from the birth of artificial intelligence is unlikely to provide a satisfactory account of the special features of human intelligence. In our Response, we begin by addressing the general comments and criticisms directed at the emerging enterprise of deictic and embodied cognition. In subsequent sections we examine the topics that constitute the core of the commentaries: embodiment mechanisms, dorsal (...) and ventral visual processing, eye movements, and learning. (shrink)

In this paper, we consider the role of conversations in contributing to healthcare quality improvement. More specifically, we suggest that conversations can be important in responding to what we call ’normative complexity’. As well as reflecting on the value of conversations, the aim is to introduce the dimension of normative complexity as something that requires theoretical and practical attention alongside the more recognised challenges of complex systems, which we label, for short, as ’explanatory complexity’. In brief, normative complexity relates to (...) the inherent difficulty of deciding what kinds of changes are ’improvements’ or, more broadly, what is valuable in healthcare. We suggest that explanatory and normative complexity intersect and that anyone interested in healthcare improvement needs to be sensitive to both. After briefly introducing the idea of normative complexity, we consider some contrasting examples of conversations, reflecting on how they do and might contribute to healthcare quality. We discuss both conversations that are deliberately organised and facilitated (’orchestrated conversations’) and more informally occurring and routine conversations. In the first half of the paper, we draw on some examples of orchestrated and routine conversations to open up these issues. In the second half of the paper, we bring some more theoretical lenses to bear on both conversations and normative complexity, summarise what we take to be the value of conversations and draw together some of the implications of our discussion. In summary, we argue that conversations can play a crucial role in negotiating the normative complexity of healthcare quality improvement because of their capacity to hold together a plurality of perspectives, to contribute and respond to emergence and to help underpin institutional conditions for empathy and imagination. One rationale policy-makers sometimes give for declining to fund a service or intervention is on the grounds that it would be ‘unaffordable’, which is to say, that the total cost of providing the service or intervention for all eligible recipients would exceed the budget limit. But does the mere fact that a service or intervention is unaffordable present a reason not to fund it? Thus far, the philosophical literature has remained largely silent on this issue. However, in this article, we (...) consider this kind of thinking in depth. Albeit with certain important caveats, we argue that the use of affordability criteria in matters of public financing commits what Parfit might have called a ‘mistake in moral mathematics’. First, it fails to abide by what we term a principle of ‘non-perfectionism’ in moral action: the mere fact that it is practically impossible for you to do all the good that you have reason to do does not present a reason not to do whatever good you can do. And second, when used as a means of arbitrating between which services to fund, affordability criteria can lead to a kind of ‘numerical discrimination’. Background: Homeless persons in the United States have disproportionately high rates of illness, injury, and mortality and tend to believe that the quality of their end-of-life care will be poor. No studies were found as to whether nurses or nursing students require moral courage to help homeless persons or members of any other demographic complete advance directives. Research hypothesis: We hypothesized that baccalaureate nursing students require moral courage to help homeless persons complete advance directives. Moral courage was defined as a (...) trait of a person or an action that overcomes fears or other challenges to achieve something of great moral worth. Research design: The hypothesis was investigated through a qualitative descriptive study. Aside from the pre-selection of a single variable to study (i.e. moral courage), our investigation was a naturalistic inquiry with narrative hues insofar as it attended to specific words and phrases in the data that were associated with that variable. Participants and research context: A total of 15 baccalaureate nursing students at a public university in the United States responded to questionnaires that sought to elicit fears and other challenges that they both expected to experience and actually experienced while helping homeless persons complete advance directives at a local, non-profit service agency. Ethical considerations: The study was approved by the Internal Review Board of the authors’ university, and each participant signed an informed consent form, which stated that the study involved no reasonably foreseeable risks and that participation was voluntary. Findings: Before meeting with homeless persons, participants reported that they expected to experience two fears and a challenge: fear of behaving in ways that a homeless person would deem inappropriate, fear of discussing a homeless person’s dying and death, and the challenge of adequately conveying the advance directive’s meaning and accurately recording a homeless person’s end-of-life wishes. In contrast, after their meetings with homeless persons, relatively few participants reported having encountered those obstacles. So, while participants required moral courage to assist homeless persons with advance directives, they required greater moral courage as they anticipated their meetings than during those meetings. Discussion: Our study breaks new ground at the intersection of nursing, moral courage, and advance directives. It might also have important implications for how to improve the training that US nursing students receive before they provide this service. Conclusion: Our results cannot be generalized, but portions of our approach are likely to be transferable to similar social contexts. For example, because homeless persons are misunderstood and marginalized throughout the United States, our design for training nursing students to provide this service is also likely to be useful across the United States. Internationally, however, it is not yet known whether our participants’ fears and the challenge they faced are also experienced by those who assist homeless persons or members of other vulnerable populations in documenting healthcare wishes. Traditional scientific views of the conscious self and world we live in are challenged by an unprecedented outburst of emerging new paradigms, theories of consciousness, perceptions of reality, new sciences, new philosophies, epistemologies, and a host of other transformative approaches. This still expanding outburst can be traced, on both logical and chronologic grounds, not to chaos theory, ecology, the new physics, or dozens of other currently ascribed sources, but rather to the cognitive (consciousness) revolution that immediately preceded. These new approaches (...) all share one key feature in common, namely, they all depend, directly or indirectly, upon a refutation and successful overthrow of the long dominant materialist paradigm. This is what was required to shift consciousness from an acausal or "eliminable" status to a functionally indispensable role. Traditional "bottom‐up" micro‐determinism had to be supplemented by a reciprocal, "top‐down" control exerted by mental emergents over lower‐level components. This does not dispose of behaviorism or reductionism, as methodology, but simply incorporates these within a more comprehensive explanatory framework. With the long reign of materialist constraints finally broken, other antireductive views have proliferated but with little regard for what the new paradigm allows. (shrink)

Background: Poverty and social deprivation have adverse effects on health outcomes and place a significant burden on healthcare systems. There are some actions that can be taken to tackle them from within healthcare institutions, but clinicians who seek to make frontline services more responsive to the social determinants of health and the social context of people’s lives can face a range of ethical challenges. We summarise and consider a case in which clinicians introduced a poverty screening initiative into paediatric practice (...) using the discourse and methodology of healthcare quality improvement. -/- Discussion: Whilst suggesting that interventions like the PSI are a potentially valuable extension of clinical roles, which take advantage of the unique affordances of clinical settings, we argue that there is a tendency for such settings to continuously reproduce a narrower set of norms. We illustrate how the framing of an initiative as QI can help legitimate and secure funding for practical efforts to help address social ends from within clinical service, but also how it can constrain and disguise the value of this work. A combination of methodological emphases within QI and managerialism within healthcare institutions leads to the prioritisation, often implicitly, of a limited set of aims and governing values for healthcare. This can act as an obstacle to a genuine broadening of the clinical agenda, reinforcing norms of clinical practice that effectively push poverty ‘off limits.’ We set out the ethical dilemmas facing clinicians who seek to navigate this landscape in order to address poverty and the social determinants of health. -/- Conclusions: We suggest that reclaiming QI as a more deliberative tool that is sensitive to these ethical dilemmas can enable managers, clinicians and patients to pursue health-related values and ends, broadly conceived, as part of an expansive range of social and personal goods. ABSTRACTTask-irrelevant emotional expressions are known to capture attention, with the extent of “emotional capture” varying with psychopathic traits in antisocial samples. We investigated whether this variation extends throughout the continuum of psychopathic traits in a community sample. Participants searched for a target face among facial distractors. As predicted, angry and fearful faces interfered with search, indicated by slower reaction times relative to neutral faces. When fear appeared as either target or distractor, diminished emotional capture was seen with increasing affective-interpersonal psychopathic (...) traits. However, moderation analyses revealed that this was only when lifestyle-antisocial psychopathic traits were low, consistent with evidence suggesting that these two facets of psychopathic traits display opposing relationships with emotional reactivity.
